Sindrom Koroner Akut (SKA) merupakan salah satu penyebab utama morbiditas dan mortalitas akibat penyakit kardiovaskular. Aktivasi trombosit berperan penting dalam proses aterotrombosis yang mendasari terjadinya ST-Elevation Myocardial Infarction (STEMI) dan Non-ST-Elevation Myocardial Infarction (NSTEMI). Penelitian ini bertujuan menganalisis perbedaan nilai indeks trombosit antara pasien STEMI dan NSTEMI di Rumah Sakit Advent Bandar Lampung. Penelitian menggunakan desain observasional analitik dengan pendekatan retrospektif terhadap 104 pasien yang memenuhi kriteria inklusi. Data diperoleh dari rekam medis dan dianalisis menggunakan uji Chi-square, Mann–Whitney, dan Independent Samples t-test. Hasil penelitian menunjukkan bahwa jenis kelamin berhubungan signifikan dengan tipe infark miokard (p=0,018), sedangkan usia pasien NSTEMI lebih tinggi dibandingkan STEMI (p=0,014). Median jumlah trombosit pada kelompok STEMI lebih tinggi dibandingkan NSTEMI, yaitu 416.500 sel/mm³ dan 310.000 sel/mm³ (p<0,001). Rerata MPV, PDW, dan PCT pada kelompok STEMI juga lebih tinggi dibandingkan NSTEMI dengan nilai p masing-masing 0,015; 0,001; dan 0,007. Disimpulkan bahwa jumlah trombosit, Mean Platelet Volume (MPV), Platelet Distribution Width (PDW), dan Plateletcrit (PCT) berbeda secara signifikan antara pasien STEMI dan NSTEMI serta berpotensi menjadi parameter hematologi tambahan dalam evaluasi pasien SKA.
