Hubungan Neutrophil-to-Lymphocyte Ratio dan Monocyte-to-Lymphocyte Ratio dengan Kadar Troponin I pada Pasien Infark Miokard Akut di RS Advent Bandar Lampung

Abstrak

Infark miokard akut (IMA) merupakan manifestasi sindrom koroner akut yang memiliki angka morbiditas dan mortalitas tinggi. Inflamasi berperan penting dalam patogenesis IMA, sehingga biomarker inflamasi berpotensi digunakan untuk menilai derajat kerusakan miokard. Penelitian ini bertujuan menganalisis hubungan Neutrophil-to-Lymphocyte Ratio (NLR) dan Monocyte-to-Lymphocyte Ratio (MLR) dengan kadar Troponin I pada pasien IMA di RS Advent Bandar Lampung. Penelitian observasional analitik dengan pendekatan retrospektif dilakukan terhadap data rekam medis 104 pasien IMA periode Januari 2024–Desember 2025. Analisis hubungan dilakukan menggunakan uji korelasi Spearman. Sebagian besar pasien berjenis kelamin laki-laki (70,2%) dan berusia ≥65 tahun (51,0%). Nilai NLR meningkat pada 73 pasien (70,2%) dan MLR meningkat pada 55 pasien (52,9%). Nilai NLR, MLR, dan kadar Troponin I pada kelompok STEMI lebih tinggi dibandingkan NSTEMI (p<0,05). Terdapat hubungan positif yang signifikan antara NLR dan kadar Troponin I (r=0,849; p<0,001) serta antara MLR dan kadar Troponin I (r=0,705; p<0,001). NLR menunjukkan kekuatan korelasi yang lebih tinggi dibandingkan MLR. NLR dan MLR berpotensi digunakan sebagai biomarker inflamasi tambahan yang sederhana, cepat, dan ekonomis dalam evaluasi pasien IMA.
