Abstrak

Hepatitis B merupakan salah satu infeksi menular melalui transfusi darah yang dapat mengancam keamanan transfusi. Metode Immunochromatographic Test (ICT) masih digunakan pada beberapa Unit Transfusi Darah, namun validitas diagnostiknya dibandingkan dengan Chemiluminescence Immunoassay (CLIA) perlu dievaluasi. Penelitian ini bertujuan untuk mengetahui validitas diagnostik metode ICT dalam pemeriksaan Hepatitis B Surface Antigen (HBsAg) dengan menggunakan CLIA sebagai metode pembanding. Penelitian observasional analitik dengan desain cross-sectional ini melibatkan 100 sampel darah donor di Unit Transfusi Darah PMI Provinsi Lampung. Hasil pemeriksaan HBsAg menggunakan ICT dibandingkan dengan CLIA sebagai gold standard. Analisis dilakukan dengan menghitung sensitivitas, spesifisitas, Positive Predictive Value (PPV), dan Negative Predictive Value (NPV). Hasil penelitian menunjukkan bahwa metode CLIA mendeteksi 53 sampel (53%) reaktif dan 47 sampel (47%) nonreaktif, sedangkan metode ICT mendeteksi 39 sampel (39%) reaktif dan 61 sampel (61%) nonreaktif. Metode ICT memiliki sensitivitas sebesar 73,58%, spesifisitas 100%, PPV 100%, dan NPV 77,05%. Disimpulkan bahwa metode ICT memiliki spesifisitas dan PPV yang sangat baik, namun sensitivitas dan NPV yang lebih rendah dibandingkan CLIA. Oleh karena itu, CLIA lebih direkomendasikan untuk skrining HBsAg pada darah donor guna meningkatkan keamanan transfusi darah.
